App hasn't worked for me for two weeks. After completing questionnaire, i cannot submit form.

The sound is awful and unnecessary. Why would I need an alert sound (mind you, it sounds way worse than an amber alert) every time I complete the questionnaire? On top of that, on my day off I was in my car and I hadn't completed it because I was not going to be at my workplace, and suddenly around 10 AM that sound started blasting through my car speakers out of the blue. I literally jumped in my seat.

The concept is fine and I appreciate having easy access to protocols, but the Covid-19 questionnaire element needs some work. After completing the questions, a confirmation notification pops up in app and makes a God-awful sound that is so unnecessary. Also, I would really appreciate an option to change my notifications - I don't need an email and a text message every time I complete the questionnaire.

Makes this horiffic loud siren noise on both phones when one of us fills out the health screening form for our kid. That has got to stop.
