Aplikasi Survey Kepuasan Masyarakat UPTD Puskesmas Mojo Kediri, Jawa Timur. Tersedia dalam dua mode survey yaitu mode reguler dan mode kuisioner. Aplikasi ini dibuat dengan mengacu pada Peraturan Menteri Pendayagunaan Aparatur Negara dan Reformasi Birokrasi nomor 14 Tahun 2017.
Feedback atau Umpan Balik sebagai respon dari masyarakat pengguna layanan akan tersimpan di database online. Dari data tersebut akan menghasilkan laporan secara terperinci mengenai Nilai Interval, Nilai Interval Konversi, Mutu Pelayanan sampai Kinerja Unit Pelayanan yang semuanya bermuara pada Indeks Kepuasan Masyarakat. Dengan menggunakan pengukuran Skala Likert dan dengan perhitungan yang komprehensif sehingga dapat menjadi acuan untuk manajemen dalam mengambil langkah-langkah pembenahan apabila terdapat hal-hal yang kurang dalam performa pelayanan yang dibagi dalam 10 Unsur Pelayanan.
Daftar pertanyaan yang disajikan dalam aplikasi ini pada mode kuisioner dapat sepenuhnya dikontrol oleh administrator dengan berpedoman pada Peraturan Menteri Pendayagunaan Aparatur Negara dan Reformasi Birokrasi nomor 14 Tahun 2017. begitupun juga dengan opsi ketidak puasan pelanggan/pengguna layanan.
