About this app

Success in the long-term control of chronic diseases greatly depends on the patients’ selfmanagement. If these patients can manage conditions such as diabetes and heart disease better outside of the hospital, costly major medical episodes are less likely to occur. While the prevalence of chronic diseases continues to be on the rise, most patients remain irresponsible, negligent, and incompliant to treatment. The rise in the mortality rates and complications due to these chronic diseases are clear proves that patients are still incapable and incompetent in selfmanaging themselves. Several studies have however demonstrated that patients will be more conscious and compliant to treatment when better assisted and accompanied by their healthcare providers.


The Medical Partner (Medpart) is an application which will permit healthcare providers to remotely monitor their patients outside of the hospital. The application could be downloaded an installed on mobile phones, tablets and computers. The Healthcare personnel will register his patients in his accounts. The patients will register their daily parameters in the application and the healthcare personnel will receive alerts about abnormal parameters. The Healthcare personnel can modify their treatment plans, invite them for a consult, send them educational material on their diseases and be reminded when the patients’ control tests are due. The application will also remind patients of their drugs, exercise and diet plans.


Generally, the Medpart will permit the healthcare personnel and the patients to work together for better control and overall success in the management of chronic diseases.
