This is a wonderful initiative and I must laud you guys for the effort. But lemme be frank, first you need to make the app accessible offline even if it's some vital information. Secondly all the data being collected is not really necessary from my view point since not everyone will want to give these data here and still give them before donating or collecting blood. Try to limit demographic data to names, contact and blood group. Lastly, if I can suggest that you guys try to work on a database and option for people to know where blood is needed to better facilitate emergency intervention since I believe that was the idea behind the app creation.
