Closeconnexions (CCX) has developed a Tele-medicine Solution that helps Physicians to monitor their Patients vitals remotely. The Patient shall conduct the tests for Vitals, and the Physician can either use the Live Monitor or the Dashboard to have a quick view of the conditions and advice accordingly. The Solution also provides Historical view of the Patients Vitals on Daily Calendar. In Addition, the Physician can conduct the Cognitive and Activity Daily Living (ADL) assessments of the Patients to track the trend of Patients Medical condition. CCX has developed few Wearable and Portable medical devices that can capture the Health Vitals of the Patients.

1. Real-time Location Tracking
The caregivers don’t have worry about the patient going out and getting lost due to memory lapse. With our real-time location tracking facility, caregivers can locate the current location of the patient, any time.
2. Geo-Fencing
Using our mobile app, caregivers can easily set up multiple geo-fences for the patients to get a notification when the patient moves in or out of the geo-fence.
3. Fall & SOS Alarm and Notification
In the event of an accidental fall or when the patient presses the SOS button for any emergency, our wearable device detects and automatically sends text message and makes phone calls to caregivers (up to 5 members).
4. Cognitive Assessment
We have built a standard cognitive assessment tool – made available through the mobile app - to enable the caregiver to assess the patient’s condition periodically. Each assessment’s score is compared with previous one, thus providing the progress of the patient to the caregivers and physician.
5. ADL (Activities of Daily Living) Assessment (under PAD: Physical Activity)
By using standard ADL measures, caregivers and family members can assess the patient’s level of independence periodically.
7. Social Help
CCX has developed a unique way to get help from other citizens In case no caregiver is available during emergencies (SOS/Fall alarm, geo-fencing crossing notifications). With the help of Closecare mobile app, opted-in citizens (with complete anonymity) who are in the same vicinity as the patient will also receive the emergency notifications that will enable them reach out to help the ones in need.
8. Smart Watch
In addition to the safety features, the wearable device doubles up as a smart watch. The wearer can use this to make phone calls, send or receive text messages, listen to music, FM Radio and reminders & alarms. For any patient who is averse to being monitored or tracked, the caregivers can share this device as a smart watch which has similar useful day-to-day used features like in smartphones.
9. Fitness Tracking
Our wearable device can monitor and measure fitness activities like Walking (number of steps, distance covered, time taken for each session) as well as track sleep. The mobile app shares the details of the fitness activities in the form of a chart and summarised report.
10. Monitoring of Basic Health Vitals
In a typical scenario, a patient who has had surgery wears the CCX device. The Patient Heart rate, Blood Pressure, Blood Glucose, & temperature levels are monitored by the healthcare staff at the Senior Citizen Care or their Care givers by connecting the CCX device to record the health Vital readings. The Data captured in real-time sent to the respective Physician's App and a Command Center for active monitoring. Danger signs like tachy/brady-cardia and hyper/hypo-thermia, with user-defined cutoffs can be detected and notified to the Care centers and patient well-being ensured.
