JoinCareTeam—connecting health providers to patients, allowing access to health care from any device, anywhere! Patients will be connected to their care team to create a plan focused on improving the health, happiness and livelihood of the patient.

Together, patients will work with their care team to create a plan of action and carefully track progress over time. Additional care team members may include doctors, nurses, health coaches, peer advocates, social workers, other health care support staff, and interpreters as needed. JoinCareTeam can support up to 4-way video calls.

Patients can get started by talking with a community health center to be provided with access to JoinCareTeam. From there, the patient will be on their way to improved health, happiness and livelihood!
