About this app

The Anticoagulant App is a module prepared to assist the Patient and
Medical Professional in monitoring and maintaining therapeutic profile of
patients on life-long anticoagulant medicines. Special care is required when
receiving these medications as it can lead to complications (stroke/
bleeding) if not administered under medical supervision.

The anticoagulant App is a pioneering patient support initiative created
by Nursing Informatics, Computer Facility and Department of Cardiology,
All India Institute of Medical Sciences (AIIMS). This APP is for in-patient
and out- patients receiving anticoagulants under AIIMS supervision, point
of care INR monitoring resulting in better patient outcomes and improved
compliance.

This APP will help maintain patient PT /INR records in real time and can be
updated remotely by patients from their homes. Self-care instructions are
also available in both English and Hindi for the patients review.
The application is a clinical decision support tool designed to guide
clinicians as they make decisions about prescribing anticoagulants for each
patient under their care.
