Sekolah Menengah Pertama (SMP) Negeri 12 Madiun merupakan alih fungsi dari Sekolah Teknik (ST) memiliki 2 lokasi, di Jl. Yos Sudarso no: 87 dan Jl Jawa no : 4.Sejak tahun 1994 terjadi alih fungsi dari ST menjadi SMP yaitu SMP Negeri 12 Madiun. Mulai tahun 2016 SMP yang terletak dijalan jawa pindah jadi satu di jalan Yos Sudarso. SMP Negeri 12 Madiun terletak dipinggir kota Madiun sebelah utara, tiga kilo meter (3 km) dari pusat kota. Tepatnya di Jalan Yos Sudarso 87, berbatasan dengan wilayah Kabupaten Madiun. Di sebelah Barat terdapat kantor dan asrama polisi , di sebelah Timur terdapat Depo Pertamina wilayah eks karesidenan Madiun, di sebelah Utara terdapat kantor pajak dan pabrik gula, serta di sebelah Selatan terdapat lingkungan perumahan dan kantor PT. INKA.

Sesuai dengan letak geografis sekolah yang berbatasan dengan wilayah kabupaten, maka profesi orang tua/ wali murid juga variatif. Ada yang berprofesi sebagai anggota TNI/Polri, PNS, petani, buruh, dan wiraswasta. Sehingga kondisi sosial ekonomi orangtua/ wali murid rata-rata berpenghasilan menengah kebawah sehingga tingkat partisipasi/daya dukung masyarakat terhadap sekolah masih belum maksimal.

Situasi keamanan sangat mendukung karena selain pagar keliling yang mengelilingi sekolah juga didukung oleh adanya kantor dan perumahan polisi yang berada didepan sekolah. Sehingga pemantauan keamanan boleh dikatakan nonstop, sehingga proses pembelajaran berlangsung dengan aman, tertib, dan lancar.

Dari kondisi saat ini diharapkan kedepan lebih meningkat dari tahun kedepan dari segala sisi. SMP Negeri 12 Madiun saat ini memiliki fasilitas antara lain ruang kepala sekolah, ruang wakil kepala sekolah, ruang staf, ruang tata usaha, ruang guru, ruang BP, ruang teori/belajar, terpasangnya tiap kelas LCD, dan terdapat 38 CCTV yang terpasang, ruang ketrampilan elektronika, ruang laboratotium IPA, laboratorium bahasa,ruang matematika, ruang UKS, aula, lapangan upacara, lapangan basket/ volley, kantin, kamar mandi/ WC siswa dan guru, mushola, tempat parkir siswa dan guru.

Dari Aspek Ketenagaan kondisi saat ini 100% tenaga pengajar semuanya sudah berkualifikasi S1 dan mereka juga mengajar sesuai bidang dengan jumlah beban jam mengajar 24 jam/minggu. Dengan demikian diharapkan ditahun mendatang lebih meningkat baik dibidang akademik maupun non akademik.
