About this app

Elderwood developed SMARTS® Express Care in collaboration with local hospitals, HMOs and insurers to provide individuals with an alternative to the emergency room. The SMARTS express care app connects providers with Elderwood’s SMARTS express care screeners, in order to divert patients from the ER or to avoid unnecessary hospitalization.
