If you see high uninstall rates, dont be suprised. But you should know the reason why. Once a user install the app, he is in need of immediate help ONLINE. And your app is requesting the user to register OFFLINE. So the user can do nothing but to remove the app which ultimately leads to high uninstalls.

date of birth function is so out of date. we can't choose the year, month and days seperately.. I was born in 1992.So I have to tap the back button of the calender several (several) times. I wish you made seperated choice for date of birth.
