It’s not easy to remember everything your care manager or healthcare provider tells you in the office. That’s where Ellie steps in. With Ellie, you’ll have 24/7 access to your care plan and educational content specific to your condition. You’ll also be able to message your care team when you need additional support.

Improve coordination in between office visits
Within Ellie, you’ll see the tasks your care team has assigned based on your condition. The app makes it quick and easy to complete these tasks and will let your care team know you are on track with your care plan. Additionally, your care team will see any health readings and data entered in the app so they can reach out if they see any readings outside of a normal range.

Better understand and manage your health
Ellie presents your health readings and data in charts and graphs to make it easy to see how you are doing and track trends. If you have a compatible connected device like a blood pressure cuff, blood glucose device, weight scale or fitness tracker, you can connect it to your Ellie app so that any data collected through the device will be automatically pulled into to your Ellie account and shared with your care team. This makes completing tasks like checking your blood pressure daily even easier!
