About this app

myCarepod is a healthcare app for patients and their care circle of family, friends and healthcare providers, to engage in telehealth-driven coordinated care. The app connects to providers’ systems and can be configured to include a health-record and tools for personalised self-management of different conditions, including assessments, care plans, IoT device connections and educational materials. Data is held securely in your healthcare providers’ system.
The app itself is free to download, but you will need a registration code to be able to use the app. This will have been sent to you by your healthcare professional.
myCarepod enables you to:
• Access and update your personal health history including personal details and special needs, contacts, allergies, problems, procedures, medications, immunizations, lab tests, appointments, and multi-media content – including images, audio, video, documents
• Engage with personalized care plans: keep track of your regular activities and get timely reminders of actions you need to perform
• Maintain health diaries for specific conditions, such as Diabetes
• Create photo & video diaries to share with your care team
• Connect to your wearable devices including Fitbit, Withings and Garmin, and see data trends from integrated health trackers
• Get feedback on your data and trends based on rules set by your healthcare professionals
• Receive and complete assessments at appropriate points in your care
• Get relevant educational material as documents, videos and URLs, when you need it
• Securely communicate with and enlist the support of your Care Circle – family, friends and your health care team.
