I agree with another reviewer. I'm not giving my insurance company, and my birthdate and signing a forms. I was hoping, as a female with AS, to be able to give ongoing data to someone who could then use or distribute said data for research. I understand needing to know, generally, a person's age and some of the other information but asking for exact birthdates and insurance company is a bridge too far. I also would have expected more questions about careers or working environment, etc
