Thank you for your interest in Campbell Integrative Family Medicine!
Currently, all new patients go through an initial 6-month membership program. We recognize that everyone has unique medical needs and personal budgets so we created the perfect membership structure so you can find the one that works for you! The total cost of joining our practice can be as little as $3,999.00 for a basic plan or as much as $8998.00 for an ALL INCLUSIVE plan. There will be a specified down payment according to the level of of plan you choose and then the remaining balance can be paid over the course of the following 5 months. The 6 month membership program fees covers all of your office visits / consultations / emails / phone calls and an initial lab panel as our gift to you for making this commitment to your health. It also includes Dr. Campbell’s personal after-hours contact information and e-mail address. Cancellation of this membership is allowed at any time with written notice. If you choose a more comprehensive program such as our GOLD plan then more services, products and labs are included.

After the 6 month period is over, you will automatically be rolled over into our “Standard” membership program* where the maintenance fees are $258.00 per month. If you do not wish to continue on this membership program, we must have a 30 day written notice of cancellation.

As a reminder, our practice does not accept, file or bill any insurance companies.
