About this app

Aster Chronic Care @ Home is a program to manage chronic diseases (Diabetes, Hypertension etc.) that will enable customers to manage their chronic diseases by use of an Intelligent Health Service Platform from home for continuum of care. The program aims to help people to manage uncontrolled Diabetes and hypertension, to live healthier, longer and quality lives. Our focus is to empower patients to take control of their health with shared responsibility. We employ the latest technology across a vast network of Clinical practitioners under Aster DM healthcare to deliver excellence across the disease management environment. The technology functions through smart monitoring devices for blood glucose and blood pressure. Reading captured by the advanced devices will be automatically updated to a mobile application on the patients’ smart phone and the health records of the care providers. These updates are monitored by well-trained paramedics through a contact center. In case of any fluctuations in the reading pattern, patients receive SMS & calls from Aster Chronic Care Specialists for better management of their health issues
