It consist of nine items including Patients name, Age, Sex, Hospital Registration number, Specialty, Ward/ Bed number, Suspected medication, rational of use of medication and description of the ADR (Figure 2) Using of the app is made very simple with the help of user friendly UI (User interface).
There are essentially four steps for completing the request.
Step1: The user will fill the reporter’s information (his information) in the side menu tab named as “Profile”.
Step2: The reporter will enter the email address and phone number of the nearest ADR Monitoring Center (AMC) in side menu tab named as “Profile”
In step 1 and 2, the information of the reporter and AMC is saved in the app database. So, the user doesn’t need to fill this information in each successive ADR reporting.
Step3: Then the user will proceed to the main ADR Reporting Request e-Form and fill the required fields. Although it is always better to the give the complete information about the patient in the form but due to want of time only the fields of “Name” and “Ward/Bed Number” are made mandatory for successful completion of the ADR Report.
Step4: The user will then send the ADR form to the AMC, where the AMC staff will follow up the reported ADR for further analysis.
The user is free to choose from variety of the methods to send the ADR to AMC. The various methods for sending the ADR Report are either by a SMS, Internet Messenger (eg. Whatsapp or Hangout) or email (gmail).
